The Pharmacy Assistant application is a testament to what can be achieved through strong partnerships. By working closely with Hyphen and Healthfirst, we have developed a tool that not only supports pharmacists but also empowers patients. This collaboration is a shining example of how value-based care can lead to better health outcomes and more efficient healthcare systems.

Enhancing Medication Adherence

One of the primary goals of the Pharmacy Assistant application is to improve medication adherence among patients. By providing pharmacists with the tools they need to effectively manage and monitor medication regimens, we can ensure that patients are taking their medications as prescribed. This leads to better health outcomes and reduces the risk of complications related to non-adherence.

Hyphen developed the Pharmacy Assistant application through a partnership with the Healthfirst Pharmacy Services team and PrimeRx and PrimePOS software, used by more than 40% of independent pharmacists in the New York City area, are utilized in 72% of interactions between community pharmacies and Healthfirst’s Medicare members.
